The perineal and genital skin is one of the most vulnerable areas in home care: moisture, incontinence, immobility, and devices combine to produce dermatitis, pressure injury, and wounds, and this same region is where the rare but catastrophic Fournier’s gangrene begins. The skilled value of this domain is distinguishing incontinence-associated dermatitis from a pressure injury — because they are treated differently — protecting the skin, and recognizing a necrotizing infection. Documentation that records “skin okay” without the exam does not prove a nurse was required; distinguishing dermatitis from a pressure injury, or recognizing Fournier’s gangrene, does.
The purpose of this module is to give the clinician the framework for the external genital and perineal skin assessment — the dermatitis-versus-pressure-injury distinction, perineal and genital wound assessment, and device-site skin surveillance — and the chartable language that demonstrates observation and assessment, skilled procedure, and teaching and training, performed with consent, privacy, and a chaperone per policy, with wound staging routed to Wounds.
