Physician or provider notification Notify the provider for a worsening dermatitis, a new or deepening perineal wound or pressure injury, a wound or fungal infection, or a device entry-site infection or trauma; document the finding, the time, the orders received, and the follow-through.
Escalate emergently and activate EMS for signs of Fournier’s gangrene or another necrotizing perineal infection, and document the closed loop.
