Endocrine and metabolic conditions — above all diabetes — are among the most common reasons patients receive skilled home health, and they generate frequent emergencies and readmissions when glucose, insulin technique, or steroid management is not closely managed. Documentation that records a single glucose value without the skilled assessment and teaching behind it does not prove a nurse was required; an assessment that interprets a glucose pattern against the patient’s individualized targets, corrects insulin technique, and prevents an emergency does.

The purpose of this module is to give the clinician the skilled framework — glucose-pattern interpretation, insulin and oral-agent management, hypo- and hyperglycemia recognition and treatment, thyroid and adrenal surveillance, and the diabetic foot screen — and the chartable language that demonstrates skilled teaching and training, observation and assessment, and management and evaluation of the plan of care. Diabetes targets are individualized; this module never assumes a single target for all patients.
