6. Mode A — complete chartable narrative
Use when metabolic status is within the patient’s individualized targets and the regimen is managed safely. Fill the bracketed blanks with the visit’s actual values; the clinical findings are not changed — the reasoning a layperson cannot supply is added. The skilled rationale supports the record when it is accurate, individualized, consistent with the plan of care, and tied to the findings, orders, and patient response documented during the actual visit.
Skilled endocrine and metabolic assessment completed. Glucose log reviewed: [pattern/range] against the patient’s individualized ordered target of [target]; [no] hypoglycemic events or significant excursions. Insulin [type/dose] drawn and injected with correct technique on observation, site rotation appropriate; meter/sensor technique correct. No hypo- or hyperglycemia symptoms; mental status at baseline; rescue [oral carbohydrate/glucagon] available and understood. Feet [intact], protective sensation [status] (see Touch/Sensation). Thyroid/steroid regimen [adherent, no over/under-treatment]. The interpretation of the glucose pattern against the individualized target and the observed technique confirm safe, controlled management and continued appropriateness of the regimen; [monitoring/insulin/sick-day/foot-care] teaching reinforced with the patient/caregiver, who [teach-back result] (cross-reference Diabetes overlay). |
