Technique

  • Review the glucose log or download the meter or sensor; identify the pattern, the timing of highs and lows, and correlate with meals, activity, and medication.
  • Observe the patient draw and inject insulin or operate the pump; check the dose against the order, site rotation, needle technique, and storage.
  • Verify meter or sensor technique — handwashing, coding/calibration where applicable, adequate sample, and result interpretation.
  • Assess for hypoglycemia and hyperglycemia symptoms and check capillary glucose when symptomatic; treat per protocol.
  • Inspect the feet for skin breakdown and confirm whether protective sensation has been tested (perform or coordinate the monofilament screen per Touch/Sensation).
  • Review the thyroid and steroid regimens and screen for over- and under-treatment symptoms.
  • Confirm the patient’s and caregiver’s sick-day rules and rescue plan, including glucagon administration where ordered.
  • Reconcile all endocrine medications and compare glucose and weight trends to baseline and to the individualized targets.
