17. Common audit and denial risks

The patterns below are the system-specific failure modes; each maps to Segment 6 (Denial Patterns).

  • A single glucose value charted without the pattern, the individualized target, or skilled interpretation.
  • Insulin or monitoring documented as a fact (“takes insulin”) without technique assessment or effect.
  • Hypo- or hyperglycemia noted without value, treatment, or follow-through (no closed loop).
  • Repetitive, cloned diabetes notes with identical wording and no trajectory toward the target.
  • Teaching documented as “educated on diabetes” without learner response or return demonstration.
  • A stable, self-managing patient charted without an articulated ongoing skilled need (no instability, teaching gap, or management rationale).
