Functional improvement

Ambulation advanced 25 ft (contact-guard) → 100 ft (supervision) over the stay — GG0170 usual performance and the discharge goal updated, care plan revised; supports Section GG discharge performance.

Acute decline, medical cause

New one-sided weakness with a sudden drop in transfer ability — assessed for stroke, BE-FAST run, transfer pathway activated; provider/representative notified; functional change and cause documented.
