3. Normal findings — documented with skill justification

Each normal line pairs the finding with the reason a licensed clinician was required and why today, and states values against the ordered parameter or the specific prior value — never bare “WNL” or “at baseline.”

Model normal 1

Section GG capture: over the 3-day window the resident performed toileting hygiene with partial/moderate assistance consistently across shifts (coded accordingly), and chair-to-bed transfers with a walker and supervision — usual performance documented from CNA flow sheets, nursing observation, and therapy notes, supporting the GG0130/GG0170 items and the PDPM function score; consistent with the ADL care-plan goal.

Model normal 2

Functional performance stable and consistent: eating set-up-only, dressing with partial assistance, walking 50 ft with a rolling walker and supervision across all shifts — usual performance captured interdisciplinarily; supports Section GG and confirms no functional decline (F676).
