2. What to assess, technique, and required data

Capture usual performance accurately over the assessment window:

  • Usual performance (not the most-independent or most-dependent episode) for each self-care (GG0130) and mobility (GG0170) item across all three shifts.
  • The type and amount of assistance and any device used; capture from licensed and non-licensed staff and from therapy.
  • Any change in function during the window and its cause, and the safety implications (falls, skin, aspiration).

MDS-supportive data

Self-care (GG0130) and mobility (GG0170) usual performance and goals are coded in MDS Section GG; the record must document usual performance across shifts so the coding — and the PDPM function score — is supported (F641). GG was restructured in v1.20.1.
