Functional decline within the window: transfers moved from supervision to substantial/maximal assistance after a fall, with a new inability to bear weight — the decline documented across shifts for GG0170, provider and therapy notified, Section GG and the CAA updated, and the care plan revised with the new assist levels and interventions. RN assessment of the change (not just the capture) is the skilled element.

Functional improvement toward the goal: ambulation advanced from 25 ft with a walker and contact-guard to 100 ft with supervision over the stay — usual performance updated for GG0170, the discharge goal reassessed, and the care plan revised; supports Section GG discharge performance.
