5. Skilled interventions and teaching

Skilled interventions

  • Capture usual performance for each GG item across shifts and disciplines (a data-collection observation).
  • Assess and manage any functional change — the skilled element — and coordinate with therapy and restorative.
  • Ensure dependent residents receive the ADL care needed to maintain function and prevent complications (skin, contractures, falls).

Teaching / training (to mastery)

  • Teach the resident/representative the functional goals and safe technique/assist levels, with teach-back.
  • Train CNAs on accurate usual-performance capture and safe ADL assistance, with return-demonstration.
