MDS & care-plan linkage Self-care (GG0130) and mobility (GG0170) usual performance and goals are coded in Section GG; the record must document usual performance across shifts so the MDS and the PDPM function score are supported (F641). Each note ties to the ADL/functional care-plan goal (baseline F655 → comprehensive F656 → revision F657); a functional decline prompts a revision and may trigger a Significant Change assessment (F637).
