Response and reassessment Document the patient’s response and the limb outcome in measurable terms — girth stable or reduced, streaking resolved, correct return demonstration of compression application — not “tolerated well.”
State the reassessment plan: the girth, node, and skin parameters to recheck, the compression plan to confirm, and the threshold for escalation. When infection or progression appears, document the revised plan and the closed-loop coordination.
