The patterns below are the system-specific failure modes; each maps to Segment 6 (Denial Patterns).

• Limb status charted qualitatively (“no swelling,” “fine”) without girth measurement or node assessment.
• An at-risk limb charted without girth comparison or infection surveillance.
• Lymphangitis or a concerning node noted without action or follow-through (no closed loop).
• Repetitive, cloned lymphatic notes with identical wording and no trajectory.
• Teaching documented as “educated on compression” without learner response or return demonstration.
• A stable patient charted without an articulated ongoing skilled need (no surveillance, compression, or teaching rationale).
