How to use this module
This is the teaching foundation for the whole guide. Read it once to internalize the documentation discipline that every body-system, overlay, and compliance module then applies. It answers the question an auditor asks of every visit — was an RN required, and does the note prove it — and it establishes the self-contained, cold-reader standard: a note must be interpretable and actionable by a nurse who has never met the patient, so “at baseline” is never the sole content of a line. The worked examples are illustrative scaffolds — fill them with the visit’s actual values; never copy them verbatim.
