The self-contained (cold-reader) rule

A note is read by whoever covers the patient next — a per-diem, weekend, or on-call nurse who has never met them. Every finding must therefore be interpretable and actionable on its own. State the actual value or observation and an objective, retrievable reference — the ordered parameter, the measurable norm, the specific prior value, or “as documented in the comprehensive assessment/POC.” “At baseline” or “unchanged from baseline” is never the sole content of a line, because a covering nurse cannot know the baseline. The test: could a nurse who has never met this patient act on this sentence?
