Categories and format

  • Vital signs — pre/post BP, HR, RR, SpO₂, temperature, pain score.
  • Functional measures — distance ambulated (ft), stairs climbed, transfer assist level, grip strength.
  • Clinical findings — edema grade (1+–4+), wound dimensions (cm × cm × cm depth), tunneling, drainage volume.
  • Lab values — glucose, INR, weight, urine output.
  • Mastery measures — teach-back percent, return-demo accuracy, reconciliation discrepancies identified.

Use directional arrows and numeric pairs — they survive every audit: RR 26→18; SpO₂ 88→94% on 2 L; BG 312→168 mg/dL after correction insulin; edema 3+→1+ at 48 hr post-diuretic; distance 12→25 ft with a rolling walker, no rest breaks; pain 8/10→3/10 thirty minutes post-PRN morphine.
