The patient or caregiver explains the topic back to the nurse in their own words. Used for warning signs, when to call, medication purposes, action plans, and dietary thresholds. Documentation includes: the specific topic; the method (verbal, handout, video); the audience (patient, caregiver, both); the mastery measure as a percent (e.g. “teach-back 80%” = 4 of 5 key points explained back); and a reinforcement plan if mastery is under 80%.

Reviewed CHF zones with patient and daughter (caregiver). Reinforced daily-weight technique (same scale, same time, before breakfast, after voiding) and red-flag thresholds (>2 lb/day or >5 lb/week). Patient and caregiver verbalized the weight threshold and the green/yellow/red meaning. Teach-back 100%. AVS provided. Will reassess understanding next visit and add nitroglycerin use if ordered.
