• Date and time of contact (e.g. “messaged NP 14:32”).
• Person and method (NP J. Smith via secure portal; PCP M. Lee via phone).
• Content of the message — SBAR or a specific request, with the relevant data.
• Response received or follow-up plan — the exact orders received, or a scheduled time to recheck.
