1.5 Skilled vs unskilled

The Medicare home-health benefit pays for skilled services. Visits documented as routine, custodial, or aide-level work get denied — even if a nurse performed them. The line between skilled and unskilled is about what was required, not who showed up.

The test

A service is skilled when it requires the knowledge, training, and judgment of a licensed professional, and would not be safe or effective if performed by a layperson, family member, or aide.
