1.2 The Medical Necessity Sentence

If a visit produces only one defensible artifact, it should be the Medical Necessity Sentence. It is the line a denial-management nurse looks for first when an unfavorable determination comes back. It is also the easiest documentation discipline to teach and the highest-leverage one to enforce.

The formula

Skilled nursing required today to [skilled action] for [RN-level reason / risk being mitigated], resulting in [measurable effect].
