Each system’s normal-findings documentation must do three things in one paragraph:
- Document the WNL findings precisely — what was assessed, what was found, in clinical detail, with the actual values.
- State the diagnosis-related reason RN-level assessment was required — the subtle changes the nurse was watching for that a non-clinician would miss.
- Tie the assessment to the plan of care — explicitly connect to ordered interventions, monitoring parameters, or risk factors.
