8. Interventions performed

  • Skilled nasal and sinus assessment and epistaxis control with firm timed pressure and positioning.
  • Cannula-skin surveillance and offloading or padding to prevent pressure injury, with coordination to Wounds if breakdown occurs.
  • Humidification and mucosal-moisture measures to prevent recurrent bleeding, especially on oxygen or anticoagulation.
  • Recognition and escalation of uncontrolled, posterior, or large-volume epistaxis and of sinusitis with cellulitis.
  • Skilled teaching of epistaxis control, cannula skin care, and humidification, to competence with return demonstration.
  • Coordination with the provider and, for oxygen management, Respiratory and the COPD overlay; apply the Anticoagulation and Hypertension overlays for bleeding risk.
