The patterns below are the system-specific failure modes; each maps to Segment 6 (Denial Patterns).

• Nasal status charted qualitatively (“okay”) without patency, bleeding, or cannula-skin assessment.
• Continuous oxygen charted without cannula-skin surveillance in an at-risk patient.
• Epistaxis noted without volume, control, or the anticoagulation context (no closed loop).
• Repetitive, cloned nasal notes with identical wording and no trajectory.
• Teaching documented as “educated on oxygen” without learner response or return demonstration.
• A stable patient charted without an articulated ongoing skilled need (no bleeding, breakdown risk, or teaching rationale).
