“Normal/at-baseline” means the nasal airway is patent, there is no active bleeding, and the skin under any cannula is intact. Each line states the finding, the comparison to baseline, and the skilled reason a nurse was required — a bare “nose okay” does not qualify.
Nares patent bilaterally without significant obstruction or congestion.
No epistaxis, or a controlled prior bleed with an effective prevention plan in an at-risk patient.
Nasal drainage absent or clear, without a purulent or bloody component.
No sinus tenderness or facial pain; any chronic symptom unchanged from baseline.
Skin under and around the cannula intact — nares, columella, and behind the ears — without pressure marks or breakdown.
Smell at the patient’s baseline without new change.
Mucosa moist with effective humidification, and the plan appropriate to continue.
