How to use this module

This module supplies the daily skilled note and the language that proves each Part A day required a licensed clinician on that day. Each model note is a structured example to be edited to the actual shift and the resident’s orders — not a phrase to copy verbatim. The four skilled bases (observation & assessment; management & evaluation; teaching & training; and covered skilled procedures) plus rehabilitation nursing are each shown with a model daily note. The universal writing rules in Foundations — the necessity sentence, quantified outcomes, teach-back, closed-loop coordination, MDS support, and care-plan linkage — apply to every day and are not reprinted here. Disease-specific days (HF decompensation, DKA, exacerbation) route to the overlays.
