The MDS items a skilled day most often supports
Section O — IV medications, IV/enteral feeding, suctioning, oxygen, dialysis, isolation, therapy (O0390), and restorative (O0500) days.
Section N — insulin, anticoagulant, antibiotic, opioid, and psychotropic days.
Section GG — usual functional performance across shifts (the PDPM function score).
Sections M / J / K / I — pressure injury and POA status; pain and falls; weight and nutrition; active diagnoses.
Care-plan linkage
End the skilled portion with the named goal it advances (baseline F655 → comprehensive F656 → revision F657). A note that floats free of the plan is both a coverage weakness (no linkage) and a survey weakness.
