One sentence per day that connects the skilled service, the licensed-level reason, and the measurable effect — and states the daily/SNF-level need. Use the formula, then replace the effect with the resident’s actual numeric outcome from this shift.

Skilled [nursing/therapy] service required today to [skilled action] for [licensed-level reason / risk being managed], resulting in [measurable effect] — a daily need that, as a practical matter, can be met only in this SNF on an inpatient basis.

Worked examples

  • Skilled nursing required today to administer and monitor IV ceftriaxone for a complicated UTI and assess the infection response, resulting in an afebrile day with clearing urine and a downtrending WBC — a daily covered skilled procedure requiring SNF-level delivery.
  • Skilled nursing required today to observe and assess a resolving pneumonia and titrate O₂ within the ordered parameters, resulting in RR 26→18 and SpO₂ 88→94% on 2 L — daily skilled surveillance and titration available only in this SNF.
