Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.
- The quantified effect of each intervention (e.g., SpO₂ 88→94%; ambulation 25→75 ft; pain 8/10→3/10).
- Tolerance and any adverse effect of an administered medication or procedure.
- Teaching effectiveness (teach-back percent or return-demo result) and any remaining gap.
- The reassessment timing and what will be re-evaluated next shift — and whether a Significant Change assessment is triggered.
