A daily skilled note is not a system exam — it is the proof of a covered day. Build it in this order so nothing load-bearing is missing:
- Lead with the skilled service and the reason it required a licensed clinician today.
- State the objective findings and the quantified outcome against the ordered parameter or the prior value.
- Add the practical-matter/daily clause (why SNF-level, why today).
- Record any coordination and the closed loop (provider + representative).
- Tie to the care-plan goal and name the MDS item(s) the note supports.
- Close with the reassessment plan and a valid, contemporaneous signature.
