13. Skilled-need justification (SNF)

SNF services qualify as skilled under the Chapter 8 bases below. The note must connect the skilled need to the diagnosis and the specific risk, AND show the need was daily and could be met only in a SNF. A generic claim does not survive review — this is the section every other reference module points to.

Observation and assessment of an unstable condition

The condition has a reasonable potential for complication or a further acute episode requiring skilled observation to identify the need to modify treatment or obtain additional care. Distinguishing stable findings from early deterioration requires nursing judgment; the potential for daily change is what makes daily observation skilled.

Management and evaluation of the care plan

When the resident’s overall condition and the complexity of the unskilled services require a registered nurse to ensure the unskilled care achieves its purpose and to promote safety and recovery, management and evaluation of the plan is itself a skilled service — needed daily when the interacting problems require continuous oversight.

Teaching and training

Complex regimens — multiple medications, devices, wound or ostomy care, diet and self-monitoring — require skilled instruction with reinforcement to competence, often across several days toward a discharge goal.

Covered skilled procedures

The Chapter 8 direct procedures (IV/IM medications and IV feeding, enteral feeding at threshold, tracheostomy/nasopharyngeal aspiration, insertion/irrigation/replacement of catheters, sterile/complex wound care, new-ostomy care, and rehabilitation nursing) support a covered day when required and delivered by (or under the supervision of) licensed personnel.
