Patient and caregiver teaching Document each teaching point with topic, method, audience, and measured mastery (teach-back percent or return-demo result), per Foundations 1.8. In the SNF the audience may be the resident, the family/representative (discharge), or a CNA (restorative carryover).
The skilled service the resident/family must manage after discharge (medications, devices, wound, diet thresholds). Warning signs and the call-provider-vs-activate-EMS distinction.
