Used when a resident’s condition has a reasonable potential for change requiring a licensed clinician to detect and respond. The skill is the surveillance and the judgment, even when the findings are stable that day.

Post-pneumonia, day 4. Skilled respiratory observation: lungs with decreasing right-base crackles, RR 20 unlabored, SpO₂ 94% on 2 L per the ordered parameter, afebrile, no increased work of breathing. Licensed assessment required to detect early re-deterioration and to evaluate the response to the resolving infection and the O₂ order; the potential for rapid respiratory change makes daily skilled monitoring necessary, available only in this SNF on an inpatient basis. Supports the Section O oxygen day and the care-plan respiratory goal; no change, skilled observation continues daily.
