Response and reassessment Document the patient’s response and the surveillance outcome in measurable terms — incision healing over a stated interval, no hypocalcemia signs, correct return demonstration of the warning signs — not “tolerated well.”
State the reassessment plan: the mass, incision, and symptom parameters to recheck, the post-operative surveillance to continue, and the threshold for escalation. When a compressive or decompensating process appears, document the revised plan and the closed-loop escalation.
