Every neck and thyroid entry should make the skilled need explicit. The medical-necessity formula combines the skilled action, the RN-level reason it required a nurse, and the measurable effect on the patient or the plan.
The formula
skilled action + RN-level clinical reason + measurable effect on the patient’s condition or plan
Worked examples
• Skilled post-thyroidectomy surveillance was required to recognize perioral numbness with a positive Trousseau sign, prompting provider notification and a calcium order that treated hypocalcemia before tetany or laryngospasm developed.
• Skilled recognition of a rapidly enlarging, firm, fixed neck mass was required to notify the provider for an urgent malignancy evaluation that would otherwise have been missed at home.
• Skilled recognition of high fever with a rapid irregular pulse, agitation, and confusion in a hyperthyroid patient was required to activate EMS for thyroid storm (cross-reference Endocrine/Metabolic).
