“Normal/at-baseline” means the neck is structurally unremarkable, the trachea is midline, and any chronic thyroid finding is unchanged. Each line states the finding, the comparison to baseline, and the skilled reason a nurse was required — a bare “neck okay” does not qualify.
Neck without new mass or asymmetry; any surgical incision healing without complication.
Neck range of motion full without pain or limitation.
Thyroid non-enlarged without a palpable nodule or tenderness, or a chronic nodule or goiter unchanged from baseline.
Trachea midline without stridor or airway concern.
No systemic thyroid symptoms of over- or under-treatment (managed in Endocrine/Metabolic).
After thyroidectomy, the incision intact and no signs of hypocalcemia, hematoma, or airway compromise.
No compressive or swallowing-related neck symptoms, and the plan appropriate to continue.
