10. Response and reassessment

Document the patient’s response in measurable terms — residual volume drained, weight change toward target, symptoms resolved, catheter patency restored — not “tolerated well.”

State the reassessment plan: the urine, catheter, weight, and mental-status parameters to recheck next visit, the expected trajectory, and the threshold for escalation. When infection, retention, or fluid overload is identified, document the revised plan and the coordination that produced it.
