“Normal/at-baseline” means urinary and renal status is at the patient’s established baseline under the current order. Each line states the finding, the baseline comparison, and the skilled reason a nurse was required — a bare “urine clear, catheter patent” does not qualify.
Voiding at the patient’s baseline frequency and adequate volume, without dysuria, urgency, or retention.
Continence and containment at baseline, with perineal skin intact.
Urine clear and appropriately colored without odor, sediment, or hematuria.
Catheter patent and draining, secured correctly, with an intact insertion or stoma site and no obstruction.
No suprapubic distension or tenderness; post-void residual within the expected range where assessed.
No local or systemic signs of infection and no change in mental status from the cognitive baseline.
Fluid balance at baseline — weight at or near target, edema stable or absent — with the current fluid and medication plan appropriate to continue.
