17. Common audit and denial risks

The patterns below are the system-specific failure modes; each maps to Segment 6 (Denial Patterns).

• Urine or catheter described in a state word (“clear,” “patent”) with no skilled assessment, infection surveillance, or comparison to baseline.
• A mental-status change charted without linking it to a possible urinary source or taking action.
• Fluid status noted (“swelling,” “weight up”) without grade, weight comparison, or a plan response.
• Abnormal findings without notification, orders, or follow-through (no closed loop).
• Repetitive, cloned urinary notes with identical wording and no trajectory.
• Teaching documented as “educated” without learner response or return demonstration, and catheter care charted as a task without skilled assessment.
