Observation and assessment

Assessing urine and catheter function, recognizing early urinary tract infection and atypical presentation in elders, identifying retention and obstruction, and interpreting fluid-balance and weight trends against baseline require the knowledge and judgment of a licensed nurse. Serial assessment of these parameters is the core of observation and assessment for this domain.

Teaching and training

Teaching catheter care, clean intermittent self-catheterization, infection and urosepsis recognition, fluid targets, and perineal skin protection — to patient and caregiver competence with return demonstration — is skilled teaching and training.

Management and evaluation of the plan of care

Evaluating whether the fluid, diuretic, and catheter plan is maintaining the patient at baseline, and coordinating revisions when infection, retention, overload, or renal decline emerges, is skilled management and evaluation of the plan of care.

Skilled procedures

Sterile catheter insertion, change, and removal; catheter irrigation and obstruction management; bladder scanning; and sterile specimen collection are skilled procedures.
