Subjective — ask

• Burning, urgency, frequency, or difficulty voiding, and any change since the last visit.
• New confusion, decreased intake, or feeling unwell reported by the patient or caregiver.
• Adherence to fluid targets, catheter care, and the medication and diuretic schedule.
• Any blood in the urine, leakage around the catheter, or absence of drainage.
