Urinary and renal problems are common drivers of skilled home health and of avoidable hospitalization — catheter-associated infections, urinary retention, and fluid overload escalate quickly in older adults, who often present atypically with confusion rather than classic symptoms. Documentation that records “urine clear” or “catheter patent” without the skilled assessment behind it does not prove a nurse was required; an assessment that recognizes early infection, retention, or a fluid-balance change and acts on it does.
The purpose of this module is to give the clinician the skilled framework — voiding and continence assessment, catheter patency and site care, infection surveillance, intake-and-output and weight-based fluid assessment, and recognition of renal dysfunction in collaboration with laboratory data — and the chartable language that demonstrates skilled procedure, observation and assessment, teaching, and management and evaluation of the plan of care.
