Scope
The documentation disciplines every body-system, overlay, and compliance module then applies. It answers the two questions a SNF record must survive — was a skilled service required on this day (payment), and did the facility meet the Requirements of Participation for this resident (survey) — and establishes the self-contained, cold-reader standard.
How to use this module
Read it once to internalize the documentation discipline that every SNF module applies. The worked examples are illustrative scaffolds — fill them with the shift’s actual values; never copy them verbatim. Where an example names a threshold, confirm it against the resident’s current orders and care plan. Two frames read every note: the Medicare coverage reviewer (Pub 100-02 Ch. 8, enforced on ADR/appeal) and the state/CMS surveyor (42 CFR Part 483, enforced with the Appendix PP F-tags). This module trains you to satisfy both at once.
