The pattern that survives

Each system’s normal-findings documentation must do three things in one paragraph:

  • Document the findings precisely — what was assessed, what was found, in clinical detail, with the actual values.
  • State the reason licensed-level assessment was required — the subtle changes the nurse was watching for that a non-clinician would miss, and why on this day.
  • Tie the assessment to the care plan and orders — a named goal, an ordered parameter, or a monitored risk.
