Benefit period (“spell of illness”)

Coverage is counted in benefit periods, not episodes. Up to 100 days per benefit period: days 1–20 fully covered, days 21–100 subject to a daily coinsurance, days 101+ not covered. A benefit period ends after 60 consecutive days out of hospital/SNF inpatient status; a new benefit period then requires a new qualifying admission. (2026 amounts: Part A deductible $1,736; days 21–100 coinsurance $217/day — verify annually.)

The reframe rule, restated

Delete every “remains homebound” sentence. Replace it with the daily skilled-need reaffirmation and the practical-matter statement: what skilled service is required today, why it required a licensed clinician, and why it can be delivered only here. A SNF note that still argues homebound has not been converted.
