Days documented as routine, custodial, or aide-level get denied — even if a nurse performed them. The line between skilled and unskilled is about what was required, not who showed up, and — uniquely to the SNF — whether the requirement was daily and could be met only in a SNF.
The test
A service is skilled when it requires the knowledge, training, and judgment of a licensed professional and would not be safe or effective if performed by a layperson, family member, or aide — and it is needed on a daily basis that, as a practical matter, can be provided only in a SNF.
