Return-demonstration — for motor skills and devices
The learner performs the skill in front of the nurse. Used for insulin injection, inhaler/spacer, glucometer, wound care, ostomy, and — for restorative carryover — the CNA performing a transfer or ambulation technique. Document: the specific skill; the steps observed and corrected; the final accuracy as a fraction/percent; and a re-evaluation plan if under 100%.

CNA performed the restorative transfer technique for return-demonstration: gait-belt placement, lock-and-scoot, and contact-guard positioning. First attempt 4/5 steps (belt too loose); re-instructed; second attempt 5/5. Licensed judgment required to verify safe carryover of the restorative program and prevent a fall during the transfer; program continued with the CNA cleared to carry it out.
The audit-defensible teaching pattern
Topic + Method + Audience + Mastery measure + Reinforcement plan. All five. When mastery is partial, the note must show what to do about it — “teach-back 50% — will re-teach next session” is acceptable; “teach-back 50%” alone is not.
