Skilled-service categories the daily note proves

Chapter 8 recognizes both direct skilled procedures and skilled observation/management. A daily note proves at least one:

  • A covered skilled procedure — e.g., IV/IM medications, IV or enteral feeding meeting the coverage thresholds, tracheostomy or nasopharyngeal aspiration, sterile or complex wound care, new-ostomy care, catheter insertion/irrigation/replacement, or rehabilitation nursing.
  • Observation & assessment of a condition with a reasonable potential for change requiring a licensed clinician to detect and respond.
  • Teaching & training the resident/family/caregiver to a level of competence.
  • Management & evaluation of a care plan whose complexity requires a registered nurse to ensure the unskilled components achieve their purpose safely.
