And because coverage does not require improvement, the point of a quantified outcome is not to show progress — it is to show that a licensed clinician measured a trajectory and exercised judgment.
The principle
Every skilled intervention should produce a documented numeric outcome — a vital-sign change, a distance, a percentage, a count, or a grade — stated against the ordered parameter or the specific prior value.
