Sources and clinical references Previous Next 1.9 Closed-loop coordination & change of condition A SNF must coordinate care across the interdisciplinary team, the provider, and — a requirement with no home-health analogue — the resident’s representative . A change in condition triggers a notification duty (F580): notify the physician and the representative. “Coordinated with PCP” alone fails, and a change that is not communicated to the representative is a survey deficiency.
Closed-loop
A coordination event with documented (1) contact, (2) content, and (3) response — or, if the response is pending, a documented follow-up time — plus the required resident-representative notification on any change of condition.
